Abstract

Maternal Mortality Rate (MMR) and Infant Mortality Rate (IMR) in Indonesia remain significant public health challenges that require comprehensive and continuous midwifery care. The continuity of care (CoC) approach is essential to improve the quality of maternal and neonatal services through sustained monitoring from pregnancy to family planning services. This case study aimed to provide comprehensive midwifery care using the continuity of care (CoC) approach for Mrs. K, a 25-year-old primigravida (G2P1A0) at 25 weeks and 4 days of gestation, in the working area of Kalikajar 1 Public Health Center, Wonosobo Regency. This study employed a descriptive case study design with a continuity of care (CoC) approach. The subject was selected because she was a primigravida requiring continuous monitoring throughout pregnancy, childbirth, postpartum, neonatal care, and family planning services. Primary data were collected through interviews, observations, physical examinations, and documentation using the SOAP format. The midwifery care was conducted from November 5, 2025, to June 6, 2026, at Kalikajar 1 Public Health Center and the patient’s residence in Sigug Village, Kalikajar District. The results demonstrated that comprehensive and continuous midwifery care was successfully implemented from the second trimester of pregnancy through childbirth, postpartum, neonatal, and family planning periods. No complications were identified in either the mother or the baby during the care process. The delivery occurred spontaneously, the baby was born full-term in good condition, the postpartum and neonatal periods were physiological, and the mother chose a three-month injectable contraceptive method according to her needs. The implementation of continuity of care (CoC) contributed to comprehensive maternal and neonatal health monitoring, supported early detection of complications, and enhanced the quality of midwifery services. In conclusion, the implementation of continuity of care (CoC) is effective in supporting comprehensive and sustainable midwifery services, thereby contributing to improved maternal and neonatal health outcomes.

 

Abstrak

Angka Kematian Ibu (AKI) dan Angka Kematian Bayi (AKB) di Indonesia masih menjadi permasalahan kesehatan yang memerlukan pelayanan kebidanan secara berkesinambungan. Pendekatan continuity of care (CoC) penting diterapkan untuk meningkatkan kualitas pelayanan maternal dan neonatal melalui pemantauan berkelanjutan sejak masa kehamilan hingga keluarga berencana. Tujuan studi kasus ini adalah memberikan asuhan kebidanan secara komprehensif dengan pendekatan continuity of care (CoC) pada Ny. K usia 25 tahun G2P1A0 usia kehamilan 25 minggu 4 hari di wilayah kerja Puskesmas Kalikajar 1 Kabupaten Wonosobo. Metode yang digunakan adalah studi kasus deskriptif dengan pendekatan continuity of care (CoC). Subjek dipilih karena merupakan ibu multigarvida yang memerlukan pemantauan berkelanjutan selama kehamilan, persalinan, nifas, neonatus, hingga penggunaan kontrasepsi. Pengumpulan data primer dilakukan melalui wawancara, observasi, pemeriksaan fisik, dan dokumentasi menggunakan format SOAP. Asuhan dilakukan pada periode 5 November 2025 sampai 6 Juni 2026 di Puskesmas Kalikajar 1 dan rumah pasien di Desa Sigug, Kecamatan Kalikajar. Hasil studi kasus menunjukkan bahwa asuhan kebidanan berkesinambungan berjalan dengan baik mulai dari masa kehamilan trimester II , persalinan, nifas, neonatus, hingga keluarga berencana. Selama proses asuhan tidak ditemukan komplikasi pada ibu maupun bayi. Persalinan berlangsung spontan, bayi lahir cukup bulan dalam kondisi sehat, masa nifas dan neonatus berlangsung fisiologis, serta ibu menggunakan KB suntik 3 bulan sesuai kebutuhan. Penerapan continuity of care (CoC) memberikan manfaat dalam pemantauan kesehatan ibu dan bayi secara menyeluruh, mendukung deteksi dini komplikasi, serta meningkatkan kualitas pelayanan kebidanan.Kesimpulan dari studi kasus ini adalah penerapan continuity of care (CoC) efektif dalam mendukung pelayanan kebidanan komprehensif dan berkesinambungan sehingga dapat membantu meningkatkan kesehatan maternal dan neonatal.
