Maternal and infant mortality rates are one of the indicators for measuring the health status of a country. Early detection efforts to overcome morbidity and mortality for mothers, infants and toddlers can be done by implementing Continuity Of Care (COC) starting from pregnancy, childbirth, postpartum, newborns, to family planning. This study aims to provide comprehensive and continuous midwifery care to Mrs. N from pregnancy, childbirth, postpartum, neonates, to family planning. The study used a descriptive method with a case study approach and SOAP documentation. Data were obtained through interviews, observations, physical examinations, supporting examinations, documentation studies, and literature studies in February–May 2026. The results of the care showed that during pregnancy the mother experienced physiological vaginal discharge and was given boiled betel leaf care to wash the intimate organs. The delivery took place normally with the implementation of Normal Delivery Care (APN) and complementary efflurage massage therapy. During the postpartum period, the mother said her breast milk production was low and was given complementary oxytocin massage. Newborn care showed normal conditions with the administration of 1 mg of vitamin K, Hepatitis B immunization, and a SHK examination. In family planning services, the mother chose a 3-month injectable contraceptive. It is recommended that staff continue to improve the quality of midwifery services on an ongoing basis, conduct early detection of danger signs in mothers and babies, provide clear and easy-to-understand IEC, support the success of exclusive breastfeeding, ensure complete postpartum and neonatal visits, and provide individual family planning counseling according to the mother's condition, needs, and preferences so that the health of both mother and baby can be optimally maintained.
